Waiver
The Right Way Lacrosse Waiver, Release of Liability, Indemnification, Consent to Medical Attention, and Statement of Physical Condition
My child has permission to participate in The Right Way Lacrosse (TRW Lacrosse) athletic training program. As the undersigned, I acknowledge and agree to be bound by each of the following:
Voluntary participation.
I understand and confirm that my child’s participation in TRW Lacrosse athletic training is voluntary.
Identification and Assumption of Risks.
I understand that serious accidents may occur during training. Participants in TRW Lacrosse athletic training occasionally sustain mortal or serious personal injuries, as well as property damage, as a consequence thereof. I understand these risks and know that my child’s participation in TRW Lacrosse athletic training may involve risk of such personal injuries and property damage, including, permanent disability and death. I understand that this Waiver and Release of Liability is intended to address all of the risks of any kind associated with my child’s participation in any aspect of TRW Lacrosse athletic training, including, particularly, such risks created by actions, inaction, carelessness, or negligence on the part of TRW Lacrosse, its directors, officers, coaches, trainers, employees, agents, volunteers, successors, or assigns. I assume all risks, known and unknown, foreseeable and unforeseeable, in any way connected with my child’s participation in TRW Lacrosse athletic training. I accept personal responsibility for any liability, injury, loss, or damage in any way connected with my child’s participation in TRW Lacrosse athletic training.
Release and Waiver.
I release TRW Lacrosse, its directors, officers, coaches, trainers, employees, agents, volunteers, successors, or assigns from any and all liability for and waive any and all claims for injury, loss, damage, or expense, including attorneys’ fees, in any way connected with my child’s participation in the TRW Lacrosse athletic training (a “Claim”), whether or not caused in whole or part by the negligence or other misconduct of TRW Lacrosse or any of the individuals mentioned above.
Indemnification.
I agree to indemnify and to hold harmless (in other words, to reimburse and to be responsible for) TRW Lacrosse and their directors, officers, employees, agents, volunteers, successors, and assigns from all Claims (including the cost of defending any Claim I might make, or that might be made on my child’s behalf, that is released or waived by this instrument) in any way connected with or arising out of my child’s participation in the TRW Lacrosse athletic training, whether or not caused in whole or in part by the negligence or other misconduct of TRW Lacrosse or any of the individuals mentioned above.
Binding Effect.
This instrument shall be binding upon my relatives, personal representatives, heirs, beneficiaries, next of kin, or assigns and shall inure to the benefit of TRW Lacrosse and their successors and assigns.
Consent to Medical Treatment.
I authorize TRW Lacrosse to provide to my child, through medical personnel of their choice, customary medical assistance, transportation, and emergency medical services should I require such assistance, transportation, or services as a result of injury or damage related to my child’s participation in TRW Lacrosse. This consent does not impose a duty upon TRW Lacrosse to provide such assistance, transportation, or services.
Statement of Physical Condition and Compliance.
My child is in proper physical condition to participate in the TRW Lacrosse athletic training and has had a physical conducted by a medical doctor in the last 12 months.
TRW Lacrosse Property
I understand that I am fully responsible for all TRW Lacrosse property when it is in my child’s possession. I understand that I am to return all TRW Lacrosse property immediately after its use, unless, I have the verified consent of a TRW Lacrosse Director, Officer, or Coach and that all TRW Lacrosse property must be returned no later than July 30 of this year. If I lose, damage, or cause an expense to TRW Lacrosse property, I understand that TRW Lacrosse shall be permitted to pursue all collection, relief, including attorneys’ fees and court costs.
Applicable Law.
This instrument shall be governed, construed, and enforced in accordance with the laws of the state of Virginia.
THIS IS A WAIVER AND RELEASE OF LIABILITY. I HAVE READ THIS WAIVER, RELEASE OF LIABILITY, INDEMNIFICATION, CONSENT TO MEDICAL ATTENTION, AND STATEMENT OF PHYSICAL CONDITION. I UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT. I AM SIGNING THIS WAIVER, RELEASE OF LIABILITY, INDEMNIFICATION, CONSENT TO MEDICAL ATTENTION, AND STATEMENT OF PHYSICAL CONDITION VOLUNTARILY.
Code of Conduct
Players, coaches, officials, parents and spectators are to conduct themselves in a manner that demonstrates respect to other players, coaches, officials, parents, spectators and fans. In becoming a member of this organization, an individual assumes certain obligations and responsibilities to the organization and its participants. The essential elements in this Code of Conduct are HONESTY and INTEGRITY. Those who conduct themselves in a manner that reflects these elements will bring credit to themselves, their team and their organization. It is only through such conduct that our organization can continue to earn and maintain a positive image and make its full contribution to our sport and community.
The following essential elements of the Code of Conduct must be followed:
1. Sportsmanship and teaching the concepts of fair play are essential to the game and must be taught at all levels and developed both at home and on the field during practices and games.
2. The value of good sportsmanship, the concepts of fair play, and the skills of the game should always be placed above winning.
3. The safety and welfare of the players are of primary importance.
4. Players should always demonstrate positive behavior and respect toward teammates, opponents, coaches, officials, parents and spectators.
5. Coaches, players, parents and spectators are expected to demonstrate the utmost respect for officials and reinforce that respect to players/teammates.
6. Grievances or misunderstandings between coaches, officials or any other parties involved with the organization should be communicated through the proper channels and procedures, never on or about the field of play in view of spectators or participants.
7. Spectators involved with the game must never permit anyone to openly or maliciously criticize, badger, harass or threaten an official, coach, player or opponent.
8. Eligibility requirements, at all levels of the game, must be followed. Rules and requirements such as age, previous level of participation, team transfers, etc, have been established to encourage and maximize participation, fair play and to promote safety.
Image Release - MINOR
I agree that The Right Way Lacrosse, is hereby granted the unrestricted and exclusive right and permission, free from approval or review, to copyright and/or use my child's likeness in all media now or hereafter known, including but not limited to, pictures and videos of my child which he/she may be included intact or in part for promotion or other commercial use.
The Right Way Lacrosse Registration Refund Policy
TRW Lacrosse has a NO REFUND POLICY and must be agreed to in order to complete the registration process. It is recommended that independent Sports Interruption Insurance is considered in case of an injury that prevents your child to participate.
